Year 2008 petition forms are for waste shipments between November 1, 2007 and December 31, 2008. You can choose a blank printable form or one that you can fill in electronically. If you choose to print a blank form you will need Adobe Acrobat Reader, available as a free download at www.adobe.com . If you choose a fill-in form you can complete it by tabbing through and entering the appropriate information, then printing it. You can save the filled-in form as a Word document. Be sure to sign your petition then mail it with the appropriate fee to the Southwestern LLRW Commission, PO Box 277727, Sacramento CA 95827-7727. If you have any difficulty, or if you need a form for an earlier year, contact the Commission at swllrwcc@swllrwcc.org.

The Commission's tax identification number is 68-0289659.

The approved petition that you will receive will show an "assigned name & code". Be sure to use it as the generator's name on all manifests.

A disposal report is to be submitted for each petition and is due no later than June 30 of the year following the petition's year. The Commission will not allow a new petition to be approved if a disposal report is overdue. Follow the same procedures as described above to complete and print a disposal report, sign it and fax it to (815) 361-3848.
